Open the attached file in evince 3.2.1.

On my system, the "5. Consignee name and complete address" section shows "GENOA CORPORATION" until you click on that field to edit it. When you do, it changes to "something else" (literally - that was some sample text).

In general, I can not edit PDF form fields and save the values properly when closing the file.

This may be related to bug 665949 and bug 663586.

This happens to me with the pdf viewer of Chrome and with Adobe Reader 9.5.1, so I'd say it's not a bug.

Can you elaborate on that? In Adobe Reader, you cannot save the form, so maybe the bug you are seeing is related to that.
