Open the attached file in evince 3.2.1. On my system, the "5. Consignee name and complete address" section shows "GENOA CORPORATION" until you click on that field to edit it. When you do, it changes to "something else" (literally - that was some sample text). In general, I can not edit PDF form fields and save the values properly when closing the file. Can you elaborate on that?
