Started happening with 0.12.5, never an issue with 0.12.0
Steps to reproduce:

1.  Play a song.
2.  Minimize rhythmbox.
3.  wait for song to end
4.  look at the notification that pops up.  It has the wrong cover art.  This only happens when a song runs out - if you manually click next, it works properly.
