I have a streaming music server at my house (icecast) that requires
authentication (http basic auth) before allowing a connection.  I tried
to add it as a radio station in rhythmbox and play it but received the
following error: "Couldn't start playback <url>: Authentication
Required".  I did not get any window requesting username/password at
all.

I am running the newest ubuntu 9.10 and rhythmbox 0.12.5

I have submitted a bug report through Ubuntu and was asked to send it upstream.
