If you launch a GNOME terminal, you'll see the terminal icon in the window
list.  If you launch more than one, you see the generic document icon. 
Unfortunately this is true for other applications.  For instance if you
launch gimp, you see the gimp icon in the window switcher.  But if you open
more than one gimp window you see the same generic document icon.

Brian, I assume this is with metacity ? How old is this build ? Has
this only started happening recently ? Could you let us know is this
is happening with the lates build ? Thanks,

I'm assuming this means with grouping, if so it's a dup of bug 77941
