Bluetooth devices which report themselves as "handsfree" don't currently have an icon.

This manifests itself in the sound panel in control center where "handsfree" devices show the missing-icon icon.

The icon filename should be "audio-handsfree[.png]" in the "devices" directory.

Some handsfree devices actually show up as audio-headset, so I would be interested to hear if those coming up as audio-device-handsfree look different to a regular wireless headset.

Only one of my Bluetooth speakers exhibits this property.  It's a cheap, unbranded Bluetooth speaker.  It shows up with the name "SP5000" and supports the A2DP and HSP/HFP profiles.

In Bluetooth parlance, handsfree (HFP) is the protocol used by low-quality earpieces such as this one:

It's the equivalent of the one earpiece with the microphone on the cable from the early Nokia days.

Headset usually has 2 earpieces (like a pair of headphones) and a microphone, like this one:

PulseAudio makes the difference between the two based on the profiles available, but I wouldn't bother trying to differentiate them.

You can have cars with those profiles. Just copy audio-headset to audio-handsfree to shut up the warnings, and we'll go fixing PulseAudio to pass along the information about the physical shape of the device.
