Updates for Korean operators.

Hi all,

This is an update for all mobile network operators in South Korea.

Changes:
* KT: They changed the name from KTF to KT in 2011(!) but it is not reflected. Also their CDMA network was shut down in 2012(!), so killed the CDMA section. (This will merely confuse the user) Added separate APN for 3G/LTE.
* LG U+: added MMS APN.
* SK Telecom: Added separate APN for 3G/LTE.
* All: There is no distinction between prepaid/postpaid APN, so I killed <plan type="postpaid"> for all operators. If this is technically incorrect, please fix this before committing.
