Using Evince 3.21.4 from Debian Sid/unstable opening the attached PDF from [1] just shows the text below instead of the form.

Some folks also had to update their Adobe Reader under Microsoft Windows to get rid of the message, and view the “intended” content.

I believe this was marked as a duplicate of the wrong bug.  The problems is not because of attachments, but because this document uses XFA forms and poppler does not support XFA forms.  See https://bugs.freedesktop.org/14265.  It's still something that has to be fixed in poppler and not evince.
