The HIG doesn't explain how an application should present the fullscreen action (appmenu vs headerbar vs hamburger menu) and how it should behave when in fullscreen mode (eg: how to unfullscreen).

I agree that it would be good to have this. One thing the pattern ought to address is touch interaction, particularly with regards to exiting full screen. This currently isn't possible in some apps (Documents, Eye of GNOME, possibly).
