Occurs with certain PDFs (see linked example[1], too big for attachment sorry). 

The annotation icon does not appear immediately (but only after text entry is completed) and appears in a displaced position from the clicked location. 

The normal right-click annotation functions can be accessed at the clicked location (invisible) but not on the displaced icon.
