pulseaudio uses orc to do the mix, and after upgrading to orc 0.4.23, mono audio played through pulseaudio become unbearably loud if volume < 100%.

Downgrade to orc 0.4.22 fix this problem.

Most probably duplicate of bug 742271
Check if patch to pulseaudio as found at http://lists.freedesktop.org/archives/pulseaudio-discuss/2015-January/022905.html fixes it.
