Youtube video increase volume every time is loaded

Every video at youtube (and posibly other HTML5 videos in other sites) increase the system volume when are played. Steps to reproduce: 1. Set a low volume level 2. Play a video in youtube (e.g. https://www.youtube.com/watch?v=7p8Prlu3owc ) 3. Change video quality a few times in the video player (e.g. between 360 and 720) 4. Notice every time the video is loaded increases general volume. This is bad, as it can cause hearing problems due to using headphones or other hearing devices and even other issues depending on use case.
