My monitors.xml is a disaster zone -- third monitor not even given a geometry -- after I change their geometry with arandr.

Some explanations on the reproducer steps would be helpful, especially if you can take some pictures of the resulting setup and artifacts you mention.

Testing with a more recent version of GNOME would also be helpful.
