For example:

$ notify-send -t 100000 bla

shows the text 'bla' for about five seconds. This is always the case, no matter how long the expire-timeout is given.

- libnotify 0.7.5-r1 (in Gentoo)
- notification-daemon 0.7.6

libnotify is passing the timeout along. It is up to the service implementation what it does with it. gnome-shell ignores it.
