Steps to reproduce:
1. View http://www.irs.gov/pub/irs-pdf/fw4.pdf
2. Examine the results via Accerciser

Expected results: Form fields present in the document would be exposed to assistive technologies.

Actual results: Form fields present in the document are not exposed to assistive technologies.

Impact: Orca's Structural Navigation features cannot be used to access forms in PDF content. In addition, Orca has no way to even inform the user that these form fields are present.

Form fields (along with links and images) will be exposed as part of the fix for bug 728475. There is a patch there for form fields (along with links and images). So marking this bug as a dup of that one.
