If i connect a second monitor to my computer and start gnome-shell, some characters and icons in the top bar are missing. In particular the letter "t" and all the tray icons that are not updated are missing (they appear on mouse over, wifi icon appears when you connect to wifi and battery icon appears when you connect the charger)

NOTE: If the second monitor is connected AFTER starting gnome-shell, everything is fine.

It seems like the issue is not hardware specific, but related to dual monitor setups.

nouveau (xserver-xorg-video-nouveau 1:1.0.2-0ubuntu3)

Graphic card is an ATI mobility radeon HD 4500/5100 Series

You're actually using the radeon drivers. nouveau is for NVIDIA chips. Which makes it a duplicate of the first bug, which seems to be radeon-only.
