I am unable to click into the fields for "Bank", "Acc. no", "IBAN", "Address", etc in the lower half of the form. I am able to click in and fill out the other fields.

Attached the test case given in the URL (to avoid broken links in the future).

It seems the form enable some fields according to a condition (check box), something evince is not doing (or poppler).

Indeed, the non-editable text fields are marked ReadOnly. There is some javascript code that changes that when you mark the checkbox corresponding to bank account. So, this bug depends on our lack of javascript support.
