I just tried this file with epdfview and it seems to be a problem with Poppler.
I cannot reproduce this with fm@thinkpad:~$ dpkg -l | egrep '(poppler|evince)' | grep '^i' | sed 's:\s\+: :g' ii evince 3.5.90-0ubuntu3 amd64 Document (PostScript, PDF) viewer ii evince-common 3.5.90-0ubuntu3 all Document (PostScript, PDF) viewer - common files ii libpoppler-glib8:amd64 0.20.4-0ubuntu1 amd64 PDF rendering library (GLib-based shared library) ii libpoppler28:amd64 0.20.4-0ubuntu1 amd64 PDF rendering library ii poppler-data 0.4.5-10 all encoding data for the poppler PDF rendering library ii poppler-utils 0.20.4-0ubuntu1 amd64 PDF utilities (based on Poppler)
Howard: Please provide poppler and evince version information.
I think bug #684989 is a duplicate of this one...
I believe I am suffering from this bug on Ubuntu 12.10 with evince 3.60 Poppler version 0.20.4. Please see: http://bugs.launchpad.net/ubuntu/+source/evince/+bug/1072129 for some PDFs producing the bug. I am going to link this report to the downstream one. Let me know if I can help further.
ppler bug https://bugs.freedesktop.org/show_bug.cgi?id=56463, which has just been fixed.
