I just tried this file with epdfview and it seems to be a problem with Poppler.

I believe I am suffering from this bug on Ubuntu 12.10 with evince 3.60 Poppler version 0.20.4. 

Please see: http://bugs.launchpad.net/ubuntu/+source/evince/+bug/1072129 for some PDFs producing the bug. I am going to link this report to the downstream one.
