[mxf] Port mxfmux/mxfdemux to 0.11/1.0

I chose the mxf plugin for my first to port to 0.11/1.0, which turned out to be a bit ambitious. I've got it compiling, but am working through runtime issues. I've created this ticket to make others aware.

Any progress? branch? patch?
I haven't worked any further on this as priorities have shifted, and releasing what I have done is held up in our release process. Hopefully I can at least get out my partial port by next week, but no guarantees unfortunately.
Well, if you can't even guarantee that or when you'll be able to release half-finished work, I think people should feel free to go ahead and port this from scratch if they feel like it ;)

Joshua, would be great if you guys could do some testing of the muxer to verify it works correctly so we can enable it.
There is a unit test, fwiw. (it doesn't pass)
