In the hyperref package of pdflatex, the Form environment has a \CheckBox command that creates a checkbox in a PDF document.  

In evince, the checkbox is visible and the mouse changes on hover, but the checkmark does not appear on click.   

The checkbox works as expected in other PDF viewer/editors including acroread, okular, and PDF-XChange Viewer.

This is a PDF Form created by latex/hyperref. The checkbox doesn't work in evince but does in acroread, PDF-XChange Viewer, okular.  Let me know if you would also like the latex file (it's only 7 lines).

I also have the same issue on evince 3.16.1 and ubuntu 15.10.
If I force the checkbox to be checked in the latex file, after compiling it I see the check mark in the PDF, but there is no way to change it within the PDF file.
