To replicate in Ubuntu Precise
System Settings > Keyboard > Shortcuts > Custom Shortcuts
Click + and enter name and command, Apply.
The prompt at the bottom says to click a row and press keys. Clicking a row pops up the command dialog again. To enter shortcut it is necessary to click at the right hand end of the highlight.

This may seem a minor issue but has caused users to think that the whole mechanism is broken as they cannot find how to set the code.

A possible improvement would be to add the keycode to the dialog where name and command are entered.  Alternatively a different way to get to the name/command dialog such as double or right click.  Or possibly just change the prompt text when on Custom Shortcuts section.  The text is correct for the predefined shortcuts.

No, I am testing Ubuntu 12.04 Beta which comes at the moment with 3.3.92.  I reported it on launchpad and it was suggested I should report it here.

Has this changed in 3.4?  If yes that I can go back to Ubuntu to see if they are planning to pick up 3.4.

A better hint would be:

"To change a shortcut, select a row on the right and hold down the keys that you want to assign. Backspace will clear the shortcut."

When custom shortcuts is selected, we can update the hint to read:

"Use the add button to create shortcut, then select it and hold down the keys you want to use. Backspace will clear the shortcut."

Given that the instructions are correct for all the sections apart from the custom shortcuts one, I'm not sure what to make of your recommendations.

"problem" reproduced in fedora 20 (3.10.2) and confirmed also applicable to HEAD.

This is fixed in GNOME 3.22's redesigned keyboard panel.
