The dialog's great, it just needs a few layout tweaks to make it really nice.

 * There's too much vertical spacing between the initial text and the list below

 * I might be wrong but the padding between the buttons, the list and the window edge seems to be a bit inconsistent. Really we ought to have the same padding there. The attached image indicates the various paddings which should ideally be the same.

 * The string reads 'Add contact data to <contact name>'. 'Data' is geeky. Let's go for something like 'Select detail to add to <contact name>:'

Made this somewhat better, but its a bit hard to synchronize the paddings as they are theme controlled.
