I don't have much info on this but a couple of times I have noticed my media-keys (play/pause) isn't properly sent to the focused window. If I watch something full screen in Totem while having Rhythmbox paused in the background I want the play/pause media-keys sent to Totem, not Rhythmbox. Might be tricky to reproduce but I experience this like twice a week.

Media keys end up as MPRIS commands so shouldn't be passed as keys to any window, as far as I know.
