Renamed aisleriot.xml to index.docbook since that's what yelp is looking for. Also added index.docbook to the Makefile so it's included in releases.

Is the renaming really necessary? All the other files can be .xml, why just this one not?

Without the renaming, Help>Contents doesn't work in Aisleriot 3.1.92.

There apparently is a specification for how the new help: system works but you'll probably have to ask shaunm about it. It's probably a lot simpler to just have index.page or index.docbook be the indexpage though.
