Activities / Applications shows double entries for several applications and those applications aren't always the same. Sometimes an application has another entry, sometimes hasn't.

Have a look at ~/.local/share/applications. You probably have tweaked your menus in a previous version, and there still are duplicate files there.

Actually some (most) of the applications have no entry in ~/.local/share/applications.
What is interesting that sometimes I have double entries, sometimes I haven't. Here is a screenshot from hours ago where you can see this issue: http://imgur.com/vIwUw . At this very moment I have no double entries and my PC hasn't been restarted, but I restarted gnome-shell. (I also suffer memory leaks from gnome-shell).
Another thing as I wrote earlier, not always the same applications get double entries.

I also have this issue.

No .desktop files in ~/.local/share/applications, but some applications are randomly duplicate in the Applications menu. The problem occurs since gnome-shell 3.0.1, no problem with 3.0.0.

I can also confirm this bug. Even when I have removed ./local/share/applications directory completely some entries in Applications menu are randomly duplicated. All applications now have single .desktop file system wide.

This is a known issue, which has been fixed in master[0]. It will be included in the next release.
