I plugged in a USB Bluetooth dongle while running GNOME Shell. The Bluetooth functionality worked and was configurable from System Settings, but the indicator did not show up until I restarted GNOME Shell with Alt+F2 "r". After restarting GNOME Shell, the icon appeared and disappeared correctly. I then restarted my computer to see if I could reproduce the bug, and I got the same behavior as I did initially before restarting Shell manually.

The Bluetooth dongle I use is this one by Targus, ACB10US1: http://www.targus.com/us/productdetail.aspx?regionId=7&sku=ACB10US1&PageName=bluetooth%20|%20Targus%20USA

Do you see anything in .xsession-errors / stdout that could be related to this?
If the Bluetooth panel is open when you insert your dongle, is it updated to reflect the new status?

Could you please also check that bluetoothd is running once you plug your Bluetooth dongle in?
