To reproduce easily:

- Open Firefox maximized
- Press alt+f2 -> r to restart the shell
- Focus back on Firefox; note that the icon near the application name at top left is missing
- Hover the missing icon, or open another application, or go to the overview and back to Firefox
- The icon appears again

This happens in 3.0, but it used to happen in .94 too.
