It appears that the overview iconview doesn't reload icons when the icon theme changes.  This can be seem for example if the settings-daemon doesn't start (due to a bug or something) and then starts later.  Initially there are many missing icons but when the icon theme settings are set later the icons don't reload.  I'd expect the icons to appear after gsd starts properly.
