A DJVU file with TOC based on page IDs

An alternate method of referencing pages inside DJVU files is using their IDs instead of page numbers. An ID is essentially a file name of a DJVU page which is preserved even inside a bundled DJVU file. TOC entries which use this type of references are properly handled in djview and okular, but ignored by evince.

See in the attachment a sample DJVU file and a djvused script (the same as attached to #619407, but with a different format used for TOC entries).
