If I adjust Banshee's per-app volume in the PulseAudio volume preferences, Banshee resets it to some stored value when the track changes.

Also, likely related, the Banshee volume control does not reflect the pulse per-app volume control directly.

I'm not sure if this is the same bug or not: using Banshee 2.6.0 on Ubuntu 12.04 but using the LXDE desktop I find the Banshee volume is being reset on every track, but it is being reset to 100% -- this is particularly bad because I use this machine to drive a B-LINK USB audio device that distorts heavily if the Banshee volume is above 65% (regardless of the system-level volume setting, I'm not exactly sure why)

gapless and auto-level are both turned off although it does not seem to matter, the volume is reset regardless of the settings.

So even if there is a way to hack the configuration so that this preset volume is 65%, I'd be happy.

I also hit this problem... and it can be terrible when using headphones :'(

Even though this bug is older, I'm going to mark this as duplicate of bug 686908 because the latter contains two patches. Testing those patches is appreciated.
