The "Paper Size" and "Orientation" drop-downs seem to be disabled in the "Print" dialog box. The same functionality is duplicated in the "Page Setup" dialog box, and those controls work. I feel that usable controls should be in the Print dialog as well.

This is true for two printer drivers I have set up on my system: HP Laserjet 8150 (postscript) and HP Color Laserjet 4500 (postscript).

This is already fixed in evince 2.29.x, the page setup options are now in the print dialog. See bug #591551
