reproduce:

1) block your simcard by typing in wrong PIN several times
2) try to connect again

nm will not ask for PUK, which is necessary and is asked by the modem to unlock the simcard again.

Workaround:
Put simcard into mobile phone or use original driver software on windows to unlock your sim

We're tracking a lot of this in bug 604551 which should fix this particular issue.
