totem --gst-debug-level=3 log

If gstreamer pulsesink is installed, but you don't have pulseaudio enabled on
your system, totem will fail to output any sound. The logs reveal that it loads
pulsesink, which fails to connect to pulseaudio. It does not fall back to alsa
instead. All other applications I use, like Rhythmbox, play sound without any
problem with alsasink.

Only by removing the pulsesink gstreamer plug-in, totem will start using
alsasink correctly.

In gconf in /system/gstreamer/default I have set audiosink, chataudiosink and
musicaudiosink all to alsasink. There is nothing which refers to pulsesink. I
have the impression that totem just ignores these settings.
