The file which caused the evince crash was the first example from pdf forms LaTeX tutorial

BTW: This bug-report is (obviously) automagically created by bug-buddy. I did not sent it intentionally (probably clicked wrong button) and I do not know how the hell bug buddy knows my bugzilla login.
