Please describe the problem:
Some icons are deprecated/replaced/unused hence they should be removed. Following is a kill-list:

48x48/apps

administration.png  -- unsure
advanced-directory.png -- unsure
apacheconf.png -- used by gnome services setting, not yet safe to remove
applets-screenshooter.png --bug:440605 soon to be deprecated
character-picker.png -- unsure
config-date.png -- unsure
config-xfree.png -- unsure
disks.png -- unsure
eyes.png -- safe to remove, been deprecated
gdm.png -- safe to remove
gnome-amusements.png  -- unsure
gnome-audio.png -- safe to remove
gnome-desktop-config.png -- safe to remove
gnome-favorites.png -- safe to remove
gnome-finance.png -- safe to remove
gnome-networktool.png -- safe to remove
gnome-pdf.png -- safe to remove, gnome-pdf is no longer maintained and should have the icon installed seperately
gnome-searchtool-animation-rest.png -- being used, should moved to hicolor

gnome-server-config.png -- unsure, still being used by gnome services setting
gnome-settings-accessibility-keyboard.png -- unsure, used by many programs

This is one of a goal of 2.24, i am not so sure if git going to use size 48 or migrate to size 32

There are cases where icons are no longer used such as gnome-setting-ui, im-*, etc. And also there are cases where icons should be moved to proper hicolor folder.

How exactly did you determine what is and is not possibly removable?

What I did so far is recursively looking for the icon names within the source code of the program. Yes, it'd be for the best if we setup a wiki on live.gnome.org and let each programs specify if they still or not use any of the pixmap icons. What do you think?

I think your assessment is wrong, and I think newer maintainers of significantly older code might not have a good idea of what icons their module is using, and so a wiki page that they have to update probably would not be very reliable either.

I've already found at least one issue with your assessment, as gnome-unknown is in fact used by gnome-panel's code, and so it is not safe to remove immediately, as you state it is.
