I suggest a Rythmbox/iTunes import style. In place of copying(importing) files into a specific F-Spot folder, just allow the user to specify the directories to import, and watch them for changes.
Modified files could be stored in a specific folder, but the originals remains in their folder.

This way the user can keep control over his folder structure and import photos the way he want. F-Spot just scan for new files at startup to add them to the data base.
