I suggest that in this case all the available podcasts should be downloaded. In iTunes there is a global preferences option that allows to configure: Manual Download/Just the newest entry/All entries. It would be even better if this would be a seperate option for each podcast.

With commit 29058ed, it now downloads all new episodes since the last update when configured to update periodically. I'm not inclined to add options to control this without a specific use case.
