in the save dialog, click on "Create Folder", then click in the filename
entry.

There is actually a worse version of the scenario :
Click on "Create Folder", then click on the "Cancel" button

Another variation: Click "Create Folder", then move the dialog temporarily because the appropriate name for the folder depends on something displayed in the window behind it. Oops!

Mac OS and (as described in bug 171416 comment 0) Mozilla both solve this problem by using a dialog for the folder name. This has the extra advantage that the dialog has a "Cancel" button.
