In the print dialog, select paper size of "Custom Size" and change the width and
height to 8.5 by 11 inches.

The paper preview shows a square piece of paper.

I can replicate.
Looks like the paper size change is not notifying the preview to update
if you change the orientation the proportions get fixed.

Changing the orientation fixes the proportions only occasionally.
