Go to applications:///, then choose File->Create Launcher.  Fill in the
details, click OK... no launcher appears.  Only (GUI) way I can get a
launcher to appear here (and hence on my menu) is to create it on the
desktop and drag it into the window.

Probably due to a fam missing. Is fam running ? If not could you test
with it ?
