I'm using Elementary OS Luna with 0.4.2-backport. There is currently no option (that I can find at least) to download / refresh the messages of an account or all accounts. The user has to wait until Geary decides to check for mail. There should be a "Get Messages" or "Refresh" button to allow the user to get messages when he desires.
Geary should pull down messages as soon as you create the account. It will in the background pull messages back to the date you specify in the Accounts Manager. If you're reading a folder and want to view farther back in time, scroll to the bottom and more should be loaded from the server. Geary remains connected to the server and should be notified when new mail arrives. If you're not seeing this, please let me know what mail service you're using and give me a better description of what you are seeing. Also know that even if there is a bug, Yorba does not maintain the 0.4.2 backport, so a fix may not be available for you. But I'm not sure there's a bug yet.

I'm using Microsoft's outlook.com with IMAP. As an example, if I send an email to myself as a test, I can a moment later hit "Get Messages" with Thunderbird and the message is retrieved, faster than Geary's automatic retrieval (which is why I thought it checked at an interval).
We've seen some other performance problems with Outlook.com's IMAP support, so I'm unsurprised you're seeing this. In any event, this is actually a duplicate of an existing ticket, so I'll mark it as such. *** This bug has been marked as a duplicate of bug 713715 ***
