Follow these steps:

1) Plug in headphones in your laptop
2) Adjust volume to 50% with gnome controls
3) Open youtube.com and play any video

The (system) audio volume is pushed to 100%. Then,

4) Lower the volume (again system volume, not youtube volume) back to 50%
5) Select any other video
6) The system volume is again re set to 100%

Repeat all the above step without headphones plugged in (using laptop integrated speakers) and the problem will not occur.
