The draft HIG currently suggests that apps should have a Help->Search menu
item, which would allow the user to search the help pages for that
application only:

Provided this recommendation remains in the HIG, it would be good if Yelp
could support it.

I guess so... I certainly haven't been/won't be logging any bugs
against apps that don't have a Help->Search menu item, as long as I
know Yelp/the help system can't support it yet anyway.

Is this enhancement requesting fully searchable help for Gnome or something more
subtle?

The current version of the Guidelines, version 2.0 does not mention "Help, Search"

and if this were to be implemented it would make sense to provide a GTK Stock
item for it to make it easier for everyone involved.

Yep, it was really requesting fully searchable help.  (In our usual "should the
HIG be achievable or aspirational?" quandary, the Help->Search item was just
commented out of the HIG until this feature became available.)

Shaun removed my bug 162431 marking it as a duplicate of this bug - but I don't
agree that it is duplicate in 100%.

Because in this bug you request fully searchable help pages by adding option to
this application specific Help menu.

While Yelp and entire help system for gnome is available via System->Help menu
entry (I mean menu on gnome-panel).

So I agree - both bugs are in fact requests for the same feature but not in the
same way.

application -> Help -> Search vs. gnome-panel -> System -> Help....
