It is not clear what the relationship between the 'never prompt or start programs' popup menus is. It might be better to display the interface as a listbox, with the  media type in the left hand columen, and the popup list of applications in the right hand column.

Lists are generally used to contain a variable number of items. Setting as wontfix. Thanks for the bug report.
