"If I drag the file and place it exactly between it's upper neighbour and itself, the file is even removed from the playlist. That's worse!

I just notice that for the removal-issue you don't even need to play the file. Just drag any file in a custom playlist to the spot between itself and it's upper neighbour."

This looks like a dupe of bug #336045 and is fixed in CVS.  Can't reproduce it with CVS, so closing as a dupe.

No idea.  There have been so many updates/changes to the playlist code since 0.9.3/0.9.3.1 (check out the ChangeLog) that it is impossible to know which one fixed this bug, it was probably a side-effect of a lot other fixes and refactoring.  It would be easier to port current CVS than backport it to 0.9.3.1, IMHO.
