gsterror.c::_gst_library_errors_init
1) has a duplicate string for
   TABLE (t, LIBRARY, SETTINGS
2) misses docs for (see gsterror.h)
   TABLE (t, LIBRARY, ENCODE
3) should it have DECODE as well?

decode errors are not fatal, we just continue playing. Encode errors are fatal
since they corrupt the stream. So 3: no.

For the rest: fixed.
