Supposing you have two annotation in one paragraph.

One annotation spans over to the beginning of a new line of text, and the second one is at the end of that line of text. When you click on the second one to add some text, you only ever get the existing text for the first annotation.

Additionally, I would expect the list of annotations in the sidebar to have a right-click menu with options like "Edit" and "Delete". And double-clicking an annotation in that list would pop up the annotation on the page. That way, at least, I would access the annotation somehow.

The right click menu part is being implemented (patch recently submitted) in bug 795631
