With gnome-shell 3.21.91, NetworkManager 1.4, and a wired connection configured with a static IPv4, when I disable/enable the network connection from the "gnome-control-center network" screen multiple times, I can get GNOME Shell's network icon to disappear. In that case, the menu entry is also non-functional to restore the network...

In the logs I see this:
