Michael, I forgot to ask in the downstream bug report. Did this happen after certain steps? When I was looking for possible duplicates than they were either too old, or without a fix.

I'm sorry Milan, it seemed random. =/  If I had to guess, a background operation went bad, but I really don't know. (If it had happened after anything in particular I would have mentioned that in my initial report.)
