Patch to enable EXTERNAL_GROUP funcitonality

The configuration allows to explicitly enable and disable the inclusion of external groups. The same should be possible for external pages. Currently this behavior is controlled by the configuration switch ALLEXTERNALS 

The attached patch adds a new configuration option and uses it instead of ALLEXTERNALS in pagedef.cpp. EXTERNAL_GROUPS defaults to true, to be backward compatible.

This bug was previously marked ASSIGNED, which means it should be fixed in
doxygen version 1.8.4. Please verify if this is indeed the case. Reopen the
bug if you think it is not fixed and please include any additional information
that you think can be relevant.
