Add Ptcl EVO

mbpi is missing the Pakistani CDMA provider Ptcl EVO; see the attached patch.

This was initially reported downstream at https://bugs.launchpad.net/ubuntu/+source/mobile-broadband-provider-info/+bug/930088.
